Acceptance of Terms

By accessing or using this website operated by Four Seasons Therapy & Wellness (“the Practice”), you agree to be bound by these Terms of Use and our Privacy Policy. If you do not agree to these terms, do not use this site or submit any information through it.

Purpose of This Site

This website is used exclusively to collect intake information from prospective and current clients of the Practice. It is not a medical portal, emergency service, or substitute for in-person clinical care. If you are experiencing a mental health crisis, please call 988 (Suicide & Crisis Lifeline), go to your nearest emergency room, or call 911.

Consent to Data Collection & Processing

By submitting an intake form on this site, you consent to the collection, storage, and processing of the information you provide, as described in our Privacy Policy. This includes the use of AI-assisted tools to help your practitioner prepare for your sessions. All such processing is performed under HIPAA-compliant safeguards, including hardware-isolated execution environments and zero data retention, as detailed in the Privacy Policy.

Accuracy of Information

You agree to provide truthful and accurate information when completing intake forms. The Practice relies on the information you submit to prepare for your clinical care. Providing false or misleading information may affect the quality of care you receive.

No Practitioner–Client Relationship from Site Use

Submitting an intake form does not establish a practitioner–client relationship. A therapeutic relationship is established only when the practitioner has reviewed your information, determined that services are appropriate, and you have both agreed to proceed with treatment.

Intellectual Property

All content on this website, including text, design, and branding, is the property of Four Seasons Therapy & Wellness and may not be reproduced or distributed without written permission.

Limitation of Liability

This website is provided “as is.” The Practice makes no warranties regarding the availability, accuracy, or uninterrupted operation of this site. To the fullest extent permitted by law, the Practice shall not be liable for any indirect, incidental, or consequential damages arising from your use of this website. Nothing in these terms limits liability for obligations under HIPAA or applicable state healthcare regulations.

Governing Law

These terms are governed by the laws of the state in which the Practice is licensed and operates. Any disputes shall be resolved in the courts of that jurisdiction.

Changes to These Terms

The Practice may update these Terms of Use at any time. Changes take effect when posted on this page. Continued use of the site after changes are posted constitutes acceptance of the revised terms.
